Guidelines for Pretransfusion Testing, 4th edition - Amendment Note

Reference is made to section B 1.1.2 relating to the requirement for patient ID validation at the time of sample collection:
The statement contained within the box in this section has raised issues where patients for sample collection may not have a wrist band. This can occur in preadmit clinics, when domiciliary collections are carried out, and in private rooms and GP surgeries.

The society's intention was to raise the Q&S bar significantly when pretransfusion samples were collected, however we do appear to have created some difficulties for both service providers and NATA inspectors.

After due consideration ANZSBT Council has decided to reword this statement as follows:

"I certify that the blood specimen(s) accompanying this request was drawn from the patient named above and I established the identity of this patient by direct inquiry and/or by inspection of wrist band, and immediately upon the blood being drawn I labelled the specimen(s)."

As per our original intention this statement should be printed by service providers on request forms used for requesting transfusion related work and signed by the collector.
